New Patients Forms

Your First Visit

Adult Registration Form  Minor Registration Form

The initial visit usually requires a half hour and is complimentary. During that time Dr. Kouvaris will conduct a comprehensive clinical evaluation. For our patients who are children, this is when we’ll determine if the time is appropriate to begin treatment, or if treatment should be delayed until further growth and dental development occur.  The initial examination will allow us to explain the best course of treatment and give an understanding of the cost involved.

On Your Way to a Great Smile

If treatment is indicated at the initial visit, your first step is the preparation of diagnostic records. This consists of impressions for study models, a complete set of x-rays, and photographs of the mouth and face.  The compiled information enables us to carefully review the nature of your orthodontic concerns and develop a treatment plan that addresses your specific needs.

Orthodontics For Your Family

For Children
The American Association of Orthodontists recommends that every child have their first orthodontic examination by age 7.  Screening children early for orthodontic problems allows for the identification of problems that are easier treated at a younger age. Taking advantage of a child’s growth , we can correct severe bite problems and reduce the need to remove permanent teeth for crowding.  This is best accomplished when some “baby” teeth are still present.

For Adults
Adult treatment for cosmetic purposes or to improve a poor bite is more popular today than ever before. Adult treatment is now more comfortable and efficient because of new technology.  The science of orthodontics has created thinner, gentler wires and increasingly “invisible” braces. We offer the latest technology including, Invisalign®, clear braces, and low friction self-ligating braces (Damon™ System).
